First step is to make an appointment for an evaluation. I will evaluate and recommend a treatment plan for you. I will follow you through treatment, talking with you and the treatment program. After you finish with the recommendations you will return to my office and will be evaluated to determine if you have complied with the recommendations or if you may need to return for more treatment. If you meet the criteria for compliance, a formal letter will be sent to your Employer.
What do I bring to the 1st appointment?
1. Name, address, phone and fax number of the employer who received the information for the positive screening.
2. Letter you received stating you had a positive screening or have the company fax the information to (386) 673-1111.
3. The fee discussed.
